UNIFORM ORDER FORM

Please fill out the order form below with your child's jersey name, number and size customization. We understand that some players have requested specific jersey numbers so please keep in mind, number selection is on a first come, first serve basis with an option to add an alternative number. Please keep in mind, uniforms are non-refundable as they are personalized and made to order.

IMPORTANT! PLEASE READ

Uniform numbers are only held for 30 days for inactive players. Once the 30 days has lapsed, that number will become available for new program players.
If you need to reorder a “replacement uniform” please order the same number unless you are ordering for a jersey number change.
